Sign up in three easy steps:

  2. Complete form in it’s entirety, type name as digital signature at bottom of form, and submit.
  3. Once we have entered your info into the system, P.A.R. will email you a link to complete registration.

Birth to 17 years old patient portal PARENT sign up
13-17 years old patient portal sign up (for adolescent to have their own, separate access)
