Vaccines will be discussed and recommended at each routine well visit. This discussion will take place regardless of your child’s vaccination status or prior refusal of vaccines.
If you choose to refuse a vaccine for your child, we will require you to sign our Refusal to Vaccinate form. This form acknowledges that we have discussed the importance of the vaccine(s) with you and you understand the risks of choosing not to vaccinate your child.
